JAMMU: In a significant medical breakthrough, AIIMS Jammu has successfully performed a complex surgical procedure—Proximal Splenorenal Shunt (PSRS)—on a 31-year-old woman suffering from Extrahepatic Portal Vein Obstruction, a serious condition that can lead to life-threatening complications. The procedure is among the first of its kind reported in the Jammu region.
The patient had a long history of recurrent hematemesis (vomiting blood) since childhood due to portal vein obstruction, which caused portal hypertension. She also suffered from hypersplenism, resulting in an enlarged spleen, low blood cell counts, recurrent anaemia, infections, skin rashes, and easy bruising.
Despite undergoing multiple endoscopic interventions, including variceal ligation and glue injections to control bleeding from esophageal varices, her condition persisted, necessitating definitive surgical management.
The surgery was performed by Dr. Sahil Sandal, Assistant Professor and Officiating Head, along with Dr. Akash Salaria from the Department of Surgical Gastroenterology. Anaesthesia support was provided by Dr. Kanika Gupta and her team.
The Proximal Splenorenal Shunt (PSRS) is a specialized procedure in which blood flow from the spleen is redirected to the left renal vein following splenectomy. This helps reduce pressure in the portal venous system, preventing further gastrointestinal bleeding and alleviating symptoms associated with hypersplenism.
Doctors reported an uneventful postoperative recovery. A Doppler ultrasound confirmed a patent shunt, and the patient was discharged on the ninth postoperative day.
Speaking on the case, Dr. Sandal emphasized that timely surgical intervention is crucial for patients with EHPVO who do not respond to endoscopic treatment. He noted that PSRS offers a durable and effective solution, particularly for younger patients.
